Pioneer Valley has immediate openings for a Case Manager looking to work in our LTC/SNF Facility in South Hadley, MA. We provide post-acute services, rehabilitative services, skilled nursing and short-term/long-term care. Case Manager Provides a range of services, including skilled nursing care, post-acute medical and rehabilitation care, hospice care services. Case Manager is responsible for facilitating, reviewing and preparing interdisciplinary plans as well as assuring comprehensive progress reports are completed; serves as liaison between patient, physician, care team members, payor, and the social worker by monitoring and communicating patient's progres and assisting with transition to the next level of care.

Job Type: Full-time

Pay: $60,000.00 - $75,000.00 per year
