The Supervisory Lead Case Manager is responsible for supervision and oversight of assigned Lead Case Managers and Case Managers. They ensure coordinated case management services for children in an on-site children’s residential facility. Responsible for the smooth and timely intake and release of all children in a residential facility and ensure all children receive necessary orientation, medical and care services. They ensure contractual requirements and client expectations are met at the On-site residential facility. Reports to the Project Director- Case Management.
All services shall be provided in accordance with established standards, principles and ethics of the profession, applicable professional specialty organizations, and the high-quality standard for which Acuity is recognized.
QUALIFICATIONS:
- Bilingual in Spanish and English (Read, Write and Speak)
- Bachelor’s degree in social work, psychology, sociology, or other relevant behavioral science
- 2 years of supervisory experience
- 5 years of case management experience with children
- 2 years of conducting in-service training
- Must be able to multi-task, be detail-oriented, be organized and have excellent verbal and communication skills
- Proficient in Microsoft applications: (Word, PowerPoint, Excel) and other common office equipment
- Clean criminal background check
- Clean Child Abuse and Neglect (CAN) or child protective services check
PREFERRED QUALIFICATIONS:
- 1 year of Supervisory experience in a residential facility
- Proficient in use of SharePoint
- Master’s Degree in social work, psychology, sociology, or other relevant behavioral science
- 2 years of experience working with the at risk, adolescent population
DUTIES AND RESPONSIBILITIES:
- Supervises the Lead Case Managers for the Shelter Department; responsible for team building, coaching, staff appraisals, and performance measures
- Oversee the on the job training for all new hires to the assigned department.
- Ensures that all time sheets are validated and approved
- Ensures all injuries and accidents are reported according to Acuity policy
- Coordinates case management and intake services for adolescents
- Supervises and monitors the intake process flow from: admissions, assessments, medical screening, badging, inventory, clothing issuance and room assignments to ensure children are provided with required services in accordance with contract policies and procedures
- Ensure children are provided with their phone calls to sponsors and relatives in accordance with contract policies and procedures
- Ensure case managers initiate and maintain direct contact with each child and the child’s family as they are being introduced to the facility and case manager
- Ensure case managers properly document through progress notes as well as all reunification efforts and contact made with the children and sponsors
- Oversee sponsor identification and first contact
- Ensure assessments are conducted in accordance with contract policies and procedures
- Report significant incidents in accordance with contract policies and procedures
- Work closely with counselors and care coordinators to ensure communication
- Maintain the database in accordance with contract policies and procedures
- Provide reports to management as needed
- Other duties as assigned by supervisor
OTHER DUTIES:
- Performs duties in a safe manner.
- Follows the corporate safety policy.
- May lead or participate in interdepartmental project groups or task forces to integrate activities, communicate issues, obtain approvals, resolve problems and maintain specific level of knowledge pertaining to new developments, new task efforts, contract awards, and new policy requirements
- Ensures the safety and security of all children as well as the assigned work area
PHYSICAL REQUIREMENTS:
- Work involves sitting and standing for prolonged periods of time.
- Ability to Ascend/descend stairs
- Ability to lift up to 15 lbs.
- Able to withstand changing environmental conditions with weather (rain, heat, lightning, and winds)
- Able to withstand and manipulate through construction areas, sports fields, etc.
