Job Summary:
Reporting to the Associate Dean of Student Care & Engagement, the Student Care Case Manager serves a key role in fostering a safe, caring environment at PLNU.
Anchored in PLNU’s commitment to a thriving student experience/model, the Student Care Case Manager supports students in their successful navigation and connection to campus resources. Specifically, the Case Manager is responsible for fostering student wellbeing through the creation of data-driven, holistic, and team-based Care Plans to support individual students throughout the PLNU undergraduate community. The Case Manager will serve as a primary contact for identifying students who need support (including mental-health issues, relationship conflicts, social adjustment, academic challenges, family concerns, Title IX measures, and more).

Job Description:
The Case Manager works closely with key campus partners to support coordinated care and advocacy for students facing obstacles in their lives. The Case Manager triages issues, provides referrals, coordinates and communicates with partners, ensures follow-up, and maintains thorough, accurate, and timely records. Additionally, the Case Manager elevates concerns to the Student Care Team, Title IX, Student Conduct, Educational Access Center, Academic Advisors, Residential Life and other campus partners as appropriate.
Duties and Responsibilities

Student advocacy

Serve as a primary point of contact for students who need support, including mental- health issues, relationship conflicts, social adjustment, food insecurity, financial needs, academic challenges, and family concerns, among others.

Triage issues; support students through mentoring and referral to resources; coordinate and communicate with partners; ensure follow-up; and maintain thorough, accurate, and timely documentation.

Assist in creating a data-driven, team-based case-management plan to support individual students.

Maintain thorough, accurate, and timely records within approved university software/systems (i.e. Maxient and Workday)

Work closely with Associate Dean of Student Care & Engagement, Student Care Team, Bias Incident Response Team, Academic Affairs, and Title IX to coordinate care and elevate issues and concerns, as needed.

Develop and maintain robust community partnerships for treatment referrals, community resources, support programs, etc.

Maintain current knowledge of student security and privacy requirements including FERPA (Family Educational Rights and Privacy Act).

Occasional evenings and weekends may be required for crisis response.

Other duties as assigned.

Periodically, this position may be asked to lead or contribute to either ad hoc or ongoing
division/campus projects outside the scope of their day-to-day activities.
Job Requirements
Minimum Qualifications
-
Bachelor’s degree in social work, counseling, psychology or related field or the equivalent combination of education and experience/training required. Master’s degree in psychology, social work, counseling, or related field preferred.

Demonstrated deep commitment to inclusive and diverse practices that recognize intersectionality and multiple identities in support of student wellbeing and development.

Experience advising, coaching and/or providing advocacy for students in a higher education environment.

Skills in judgment and decision-making, problem solving, identifying measures of system performance and the actions to improve performance.

Demonstrated experience in problem-solving, decision making, conflict resolution, negotiation, counseling, referral techniques and confidentiality.

Candidates should be team oriented and demonstrate adaptability, initiative, and the ability to respond effectively to crises.

Excellent written, oral, and interpersonal communication skills.

Strong organizational skills with ability to handle several projects simultaneously.

1-3 years of professional experience in higher education, social work, or other related field.

Demonstrated commitment to diversity and anti-racist practices.

Direct experience providing advocacy and support for underrepresented students in a higher education setting.

Evidence of Christian commitment and active church involvement. In addition, staff agree to live in agreement with PLNU’sCommunity Life Covenant.

Preferred Qualifications

Physical Requirements:

Work is primarily sedentary and may require sitting for extended periods of time.
