Position Summary: Provides Care Coordination services to clients who are deemed eligible by CFBHN by engaging clients immediately prior to successful discharge from the CSU or Detox treatment programs. Also provides Case Management services to clients who are referred but do not qualify for Care Coordination services. Develops a care plan/needs assessment based on needs of the client to sustain stability such as: medical care, mental health services, transportation needs, housing options, vocational counseling, etc. A holistic and strength-based approach will be utilized to provide individualized and integrated care for individuals recovering from serious mental health, substance use disorders, or co-occurring substance use and mental health disorders. Assists individuals in navigating complex systems to address all identified needs and maintain their mental stability and recovery.

Educational Requirements: Bachelor’s Degree in a behavioral science or related field.

Special Skills: Basic knowledge of substance abuse and mental health treatment; working knowledge of community resources; engagement, motivational, and advocacy skills; strong interpersonal/communication skills and independent work ethics to foster the human service needs of our clients promoting cost-effective interventions and outcomes. Strong computer skills for electronic health record documentation.

Position Duties:
  • Identifies client strengths, needs, abilities, and challenges.
  • Develops initial coordination plan with client to address behavioral health issues, medical, social, housing, interpersonal problems/needs that impact the client’s status.
  • Establishes relationships with community agencies and resources
  • Makes appropriate referrals and link clients with appropriate services, treatment providers, and resources
  • Monitors client’s treatment and progress toward established goals
  • Advocates for the needs of the client
  • Documents all activities including progress or lack of progress of the case plan goals in progress notes in accordance with FSOS policy and contractual guidelines
  • Accurately completes required paperwork procedures
  • Maintains customer confidence and protects operations by keeping client information confidential; following established confidentiality policy and procedures; maintaining professional boundaries with clients
  • Computer Literacy
  • Completes all training requirements in a timely manner
  • Maintains inter- and intradepartmental workflow by fostering a spirit of cooperation.
  • Completes all other duties as assigned
