Job Summary: Under general supervision, the discharge planner will work alongside the treatment team with assisting the individual with appropriate discharge planning to include but not limited to: stepdown services, community resources, medical appointments and disposition location.

Essential Functions: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Records services accurately in the EMR and within appropriate timeframes.
  • Completes required documentation according to agency standards.
  • Attend morning staffing meeting, appropriate treatment team and document in the ECR the treatment team notes which include: any risk behaviors identified, change in LOC, decompensation, goals, discharge plans and any recommendations.
  • Complete discharge DLA on day of discharge noting changes in improvement. (Glynn site specific)
  • Make follow up appointments for any appropriate appointments the discharging individual may need such as medical, mental health, prenatal, dental, etc. Appropriate appointment sheets will be utilized for appointments allowing the individual to be provided with location, time, who they will be seeing and appointment time.
  • Make collateral calls by speaking with family, friend, or identified support person with an ROI in place, asking such question as the following: will they continue to be supportive of the individual’s treatment; any safety concerns, is the home free of drugs and weapons, and is there anything the treatment team needs to know in regards to the individual’s treatment? Will they be supportive regarding follow up care?
  • Will complete ROIs appropriately and as needed for collateral contact and discharge planning purposes.
  • Will make referrals as appropriate to such facilities as the ACT team, Georgia Regional Hospital, Homeless outreach, and appropriate long term facilities such as a half-way house.

** C&A CSU referrals to PRTF level of care if appropriate.

  • Will make APS and CPS reports as directed by the treatment team when abuse or neglect is suspected.
  • For OBS and or CSC assigned individuals, ROIs will be completed; discharge planning began or completed for OBS individuals. All documentation completed before end of day and properly billed in the EMR.
  • Provide high risk list to appropriate Outpatient site on the assigned date.

Required Education & Experience: Crisis experience is preferred and some case management experience but not required.
