Position Summary:
The Care Management Director provides the primary leadership for care management staff and Care Coordination program administration across Lawndale Christian Health Center. The Care Management Director is responsible for broad-level program management, staff operations and productivity, and oversight and implementation of new projects related to care coordination, programming, and health equity in collaboration with clinical and administrative leaders across the organization.
Principal Duties and Responsibilities*
Program Management and Administration
- Directs the Care Management programs and staff across LCHC who are responsible for offering direct patient support, education, and assistance with a focus on social services, population health, and the social determinates of health.
- Responsible for the oversight and administration of federal, state and private grant-based programming and their corresponding deliverables. Works alongside the Development Department on grant specific reporting requirements, audits and program performance.
- Monitors and develops new quality and productivity dashboards and provides direct feedback as well as support to clinic-based Site Managers to support care management responsibilities as necessary to accomplish program and organizational goals.
- Lead the development of new workflows for new projects and programs related to Care Management including care coordination, community health workers, and other social service support.
- Support and collaborate with data management and Electronic Health Records teams to ensure effective and efficient processes to collect program performance metrics and ensure they are adequately reported to external systems for program compliance.
- Serves as a primary liaison on behalf of LCHC to partners and funders including participation in various committees and workgroups related to Maternal Child Health, the West Side Health Equity Collaborative, Medical Home Network, and Ryan White HIV program.
Staff Performance and Operations
- Ensures Care Management staff are properly trained, integrated within their care teams, and productive in their outcomes and quality of work. Works in collaboration with the Clinical Complex Care Director and Care Coordination Manager to ensure staff have adequate social services training and skills to engage complex patients.
- Provides direct supervision to Care Coordination Manager and shared supervision for all Care Managers and related team members.
- Collaborate with the quality, transitions of care, clinical departments to create systems and strategies that maximize clinical outcomes.
Clinic Integration and Implementation
- Establishes and recommends systems to resolve complex barriers to patient care alongside of clinical teams.
- Designs and implements educational interventions to reduce the impact of chronic diseases and co-morbidities as well as health support for areas such as Maternal Child Health, Substance Use Disorder, HIV/AIDs and more.
- Makes connections with external social service systems to ensure smooth referrals as needed.
- Performing other duties as required to assist LCHC in achieving its mission
Required Skills or Abilities:
- Interpersonal communication and mediation skills to successfully lead, motivate, and collaborate with a very diverse staff in a variety of capacities throughout the organization.
- Exceptional organizational skills for ensuring the completion of a large volume of work in a systematic manner.
- Initiative and creativity for problem solving and pro-active improvement of staff engagement and clinic workflows
- Capable of exemplifying the values of Lawndale Christian Health Center in all circumstances.
- Strong time management skills and experience with directly supervising and managing staff.
- Embraces and personifies the mission of Lawndale Christian Health Center, by performing work in a whole person way, providing for the physical, emotional, and spiritual needs of patients
Required Knowledge, Experience, or Licensure/Registration
- At least 7 years’ experience in community health, care coordination, case management of patients of various risk groups and chronically ill patient groups.
- Three (3) years of managerial experience in a healthcare setting with deep understanding of best practices of collaborative management processes required.
- Master’s Degree in Social Work or Nursing Degree from an accredited university and Illinois State Licensure strongly preferred.
- Deep knowledge of the major functional areas of health systems operations, including all clinical, health support, and administrative departments.
- Bilingual in English and Spanish a plus.
