Job Summary

Completes birth certificates, paternity acknowledgements fetal death certificates and inducted termination forms for Greenville Memorial Medical Center and other associated Greenville Hosptial System facilities as directed by South Carolina Department of Health and Environmental Control (DHEC) and Department of Social Services Parenting Opportunity Program (DSS-POP). Works independently to complete tasks.

Accountabilities

Educates patients of newborns on the birth certificate process and provides a birth certificate worksheet for completion by the patient (Mother's Worksheet). - 20%

Abstracts detailed information from the medical record of the mother and newborn to meet DHEC requirements for completion of the Facility Worksheet. This includes, but is not limited to, prenatal care, prenantal problems, delivery process, medications, complications of delivery, newborn complications and newborn outcome. - 20%

Keys information from Mother's Worksheet and Facility Worksheet into the SC Electronic Birth Certificate system. Ensures accuracy of information keyed. - 10%

Educates patients on the Paternity Acknowledgement process as governed by DSS - POP and DHEC. Completes paperwork for requested paternities, verifies identification and notrizes form upon completion. Keys information into SC Birth Certificate system. Verifies accuracy of information entered. - 10%

Abstracts information from patient record and completes Fetal Death reports as well as induced termination reports to meet DHEC requirements. Information abstracted includes, but is not limited to, prenatal care, prenatal problems, delivery process, medications, complications, cause(s) of fetal demise. - 10%

Provides information for Stillborn Birth Certificate to patient. Types Stillborn birth certificate and provide a copy to the mother. Submits a copy to DHEC to maintain. - 10%

Knowledgeable of SC laws that govern the birth certificate process and stays updated on changes to the laws, procedures and electronic reporting system changes. - 5%

Provides timely and courteous assistance to patients following PlanTree guidelines. - 5%

Communicates effectively and courteously with DHEC personnel to assure timely and accurate correction to birth certificates when needed. - 5%

Supervisory/Management Responsibilities

This is a non-management job that will report to a supervisor, director or executive.

Minimum Requirements

High School diploma or equivalent

1 year - Health Care setting that involved patient contact

In lieu of the experience and/or educational requirements indicated above a combination of education, training, and/or experience will be considered an equivalent substitution: RHIT/RHIA or Graduate of an accredited Health Information Technician or Management Program and eligible to sit for the RHIT or RHIA exam.
