Organization:
SCAH-Sutter Care at Home - BayPosition Overview:
Provides psychosocial counseling to individuals, family members and/or other groups after the loss of a loved one. Ensures bereavement services are provided in accordance with the interdisciplinary plan of care. Facilitates grief therapy and ensures resources are available to individuals, families and/or groups in various settings including telephonically, at hospitals and other locations including churches.Job Description:
EDUCATION:
- Equivalent experience will be accepted in lieu of the required degree or diploma.
- Master's: Social Work or related field
TYPICAL EXPERIENCE:
- 2 years recent relevant experience.
SKILLS AND KNOWLEDGE:
- Understanding of psychosocial implications of illness, hospice and/or home care death and dying issues and dynamics of grief.
- Knowledge of Medicare, accrediting bodies, Community Healthcare Access Program (CHAP), state and all other regulations applicable to Bereavement programs.
- Knowledge of community resources available to bereaved persons.
- Ability to communicate effectively orally and in writing.
- Must be able to discern sensitive issues and respond appropriately.
- Must be sensitive to the human needs and situations encountered in the provision of hospice and bereavement care.
- Must have organizational skills, interpersonal skills, problem-solving abilities, ability to prioritize work.
- Ability to work under time-pressure and changing priorities and be able to deal with potentially conflicting demands.
- Must be able to handle sensitive issues, conflict with or among others, respectively direct and reinforce staff efforts; effectively plan and organize and prioritize work, and to effectively solve unique problems as they arise or identify when to consult supervisor.
- Demonstrate self-directed learning to meet professional development and/or license requirements.
- Must be able to work effectively with a minimum of supervision.
- Ability to use essential applications and/or databases associated with the role’s duties and responsibilities.
- Must be able to travel to multiple program sites.
Pay Range: $45.79 – $61.81 / hour
The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
