The Associate Behavioral Health Consultant (A-BHC) is an integral part of the primary care team to identify, treat, triage, and manage primary care patients with medical and/or behavioral health challenges. The A-BHC seeks to enhance the delivery of behavioral health services in the primary care setting and to support a smooth interface between primary care and specialty services (MH and SA treatment) under the supervision of the Behavioral Health Consultant.
JOB RESPONSIBILITIES:
- Accessible and visibly present to members of primary care and BH team during clinic hours.
- Available to primary care patients and health professionals on a same day basis to provide the consultative services within the scope of practice including client screenings and interventions.
- Develops and follows a measurable, time limited treatment plan in conjunction with the primary care provider, BHC, and the patient.
- Provides triage and patient flow assistance to the BHC.
- Facilitates primary care provider and patient requests for same-day and future consultations with the BHC/A-BHC.
- Provides requested interventions related to chronic disease management.
- Assures availability of patient education and other materials related to BHC work.
- Places referrals to specialty health and community services.
- Maintains up-to-date contact information about commonly used community resources.
- Consistently meets position productivity goals.
- Participates in a team management approach to patient care.
- Maintains appropriate clinical documentation in a timely manner.
- Participates in continuing education to maintain professional standards.
- Participates in Clinical Supervision to ensure maintenance of practice within scope of an Associate BHC.
EDUCATION
Minimum: Bachelor’s degree in social work, psychology, or other behavioral health related field
Preferred: Master’s degree in Social Work, Psychology, or related field
EXPERIENCE
Minimum: 2+ year experience in a related field
Preferred: 3 years’ experience in health care
MS Office proficiency, data entry skills- Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, and supervisors.
- Demonstrate excellent written and oral communication skills.
- Must demonstrate critical thinking, problem- solving and organizational and time management skills.
- Bilingual- Spanish desired
CERTIFICATIONS/LICENSURE
Arizona Fingerprint Clearance Card required.
