Madrid Home is looking for an Activities Supervisor.

Key Responsibilities:
Resident Programming

• Develops, implements and revises an activity service plan in conjunction with the Administrator
• Establishes program and resident activity objectives and goals
• Reviews all resident charts at least quarterly to assist in improving the service plan based on resident need

Resident & Family Relations

• Provides information for service plan meetings with residents and families
• Builds positive relationships with staff, residents, volunteers and their families
• Presents education sessions for residents to promote the benefits of recreation

Programming Compliance

• Ensures Activity Staff maintains accurate participation records and charting on resident progress toward individual programming goals
• Assures compliance with federal and state regulations concerning the Activity Department

Requirements:
Education, Experience and Training

• Associates Degree
• Social Services (Preferred)
• Prefer a minimum of two-years’ experience with specialized dementia programming in an activity department of a
• long-term care facility.
• Experience with developing Leisure plans for seniors.
• Valid lowa driver's license and willing to obtain chauffeur's license.
• Good driving record that meets insurance company requirements.
• CPR certified or willing to achieve same.
• Must be able to read, write and speak fluent English.
• Highly motivated and self-directed.
• Organized, patient and flexible
